Back To School Forms
Each year, families must complete back to school forms that include permissions for district technology use, updated health information, acknowledgement of the Student Handbook and Code of Conduct, and updates to other important information so the district is able to stay in contact with families and support student needs.
These forms are available in the Skyward Family Access account. If a student has multiple families attached to their account, only the Family 1 parent or guardian can access the forms.
