     COVID Vaccines will not be given at the same time as either Shingrix or Pneumonia vaccines as each of these have high rate of local reactions. 
     
    Enter all information, including employee ID (EP number) - not necessary for family members.
    • Select all the vaccines you will receive. 
    • Print the consent form from the confirmation page or e-mail confirmation.
    • Print ONE consent form and bring it filled out to the clinic with you. Be sure to include insurance information if you have Aetna, Blue Cross Blue Shield, Cigna, Humana or UnitedHealthCare PPO.
    • Repeat these instructions for all family members who will receive vaccinations.  Employee ID not necessary for family members.
