BEWARE - HMO - Baylor Scott & White Health Plan is an HMO plan utilizing the very small Baylor Scott & White HMO Network.  You MUST select a doctor/facility among the in-network providers for the plan to pay ANYTHING.  You and your covered dependents are only covered in the D/FW area.  Out-of-network benefits are NOT available.  Referrals are not necessary.
