Medication Information for the 2025–2026 School Year
Hebron Valley Elementary – Nurse’s Office
As we prepare for the upcoming school year, please review the following important information if your child will require medications at school.
Medication Drop-Off
Families may drop off student medications at Hebron Valley Elementary on the following dates:
August 4th, 7th, or 8th
8:00 AM – 3:00 PM
Our nurse will be available to answer questions and help ensure all required documentation is completed properly.
Dropping off medications during this time will help us stay organized and avoid long lines on the first day of school—August 12th, 2025.
Required Forms
Please complete the applicable forms before medication drop-off. Forms are linked below for your convenience:

For daily, PRN (as-needed), or asthma medications. - LISD Allergy Action Plan for Elementary Students (PDF)
For students with life-threatening allergies. - TEA Seizure Action Plan (PDF)
For students with a seizure diagnosis.
Medication Reminders
- All medications must be in the original, properly labeled container.
- Please check expiration dates—medications must be valid through the end of the school year.
- Inhalers, EpiPens, and emergency medications should be clearly labeled and in working order.
