Pre-Registration for NEW STUDENTS to Liberty Elementary School.

To complete the registration process, please provide the following required documents to the Campus:

• Certified copy of the child’s birth certificate
• Child’s current immunization record signed/stamped by physician
• Child’s Social Security Card (if available)
• Driver’s license of parent/legal guardian enrolling child
• Proof of residence for Liberty school zone (gas, water or electric bill only)

All original documents will be copied at the time of registration and returned to the parent/guardian.
