Signed request from parent is required for the School Nurse to give any medication(s) at school. Long-term medications require Doctor Orders also. Emergency rescue medications can be self-carried by the student if a Self-Carry Agreement completed by parent, physician, and student is filed with the school nurse. All orders need to be updated every school year.
