Back-to-School Forms | Aug. 4
Please review the following important information, and fill out the annual Back-to-School forms.
The district’s online back-to-school forms packet is ready for families to complete in LISD Skyward Family Access. Families of new students to the district must complete the online forms packet. Families of returning students will need to review and edit the forms as needed. Forms must be submitted for every student each school year. Included in the forms is the link to the application for free and reduced lunch.
Families who are eligible for district technology will indicate in the back-to-school forms whether or not they need an LISD device for the 2020-21 school year, or if they need to return an LISD device. There are various device options available depending on your child’s grade level. Device pick-up begins Thursday, Aug. 6 and families will schedule their pick-up time within these forms.

The back-to-school forms also include LISD’s Acceptable Use Policy for technology devices and systems. With the district beginning the school year in remote learning, it is important for us to receive device information as soon as possible to be able to provide technology needs for our students so they are prepared to begin learning on Aug. 19.
The district and your campus will utilize Skyward’s Skylert tool to communicate important information during the school year. It is important you provide up-to-date contact information in Skyward (Step 2 in the back-to-school forms packet). As a reminder, always reference your child’s campus website and social media accounts for immediate information.
If you have any difficulty accessing your Skyward Family Access account, please contact the registrar at your campus for assistance. There are also links on the Family Access login page to reset your password or submit a support ticket.

Families have until Aug. 5 at 11:59 p.m. to modify their Learning Commitment Form selections for the first nine weeks of school. To modify your selection, follow these editing instructions and make the change in Skyward Family Access.
In-Person: This option is for students who DO want to attend school in-person when campus doors open (Sept. 8). Instruction will take place remotely at home beginning Aug. 19.
Virtual (and/or Virtual Plus for secondary students): This option is for students who DO NOT want to attend school in-person when campus doors open (Sept. 8). Virtual PLUS middle school and high school students will take some elective courses, which are not offered virtually, In-Person.

*Please note - the learning option choice lasts for a 9-week grading period, and students cannot change their learning path during the middle of a 9-week grading period.
