To try and help ease the load of the call center and give families another option to report positive COVID cases, Lewisville ISD is launching an online COVID self-reporting form. Families can now use the form or call the COVID Call Center to report a positive COVID test.

In order to complete the form, families will need to know some student-specific information, including student ID, home room teacher for elementary grades, extra-curricular activities, dates of a positive test and/or exposure, and more. You will not be able to complete the form without this information.

Once the form is completed, a health plan that includes the date your student can return to school will be emailed to you and your student’s school.
