• COVID Leave

    The LISD Employee COVID leave benefit ends on June 302023.  Applications for leave will be accepted through July 7, 2023, for COVID illness related absences up to June 30, 2023. 

    COVID leave only applies to a staff member’s illness, not a family member, and will be retroactive back to July 1, 2022 through June 30, 2023. Staff members with a positive COVID test may return to work after completing five days of isolation from the date symptoms began, if the symptoms are improving and they have been fever free for 24 hours.


  • STAFF REPORTING & REIMBURSEMENT

    All forms require you to use your LISD Google account. You will use your username@staff.lisd.net and your district password to login to Google. If you are denied access to any of the forms below, it means you are not signed into Google using your LISD account credentials. Personal devices often default to a personal Google account. The LISD Intranet is your LISD username without @staff.lisd.net on it.

    If you have tested positive for COVID, please complete the four steps below to be reimbursed.

    1. Submit a SELF-REPORT FORM

    Complete the form below; you and your supervisor will receive your personalized COVID Action Plan. This plan includes days that you need to stay home due to COVID illness.  If you intend on filing for COVID leave, you MUST have a lab confirmed test; home tests are NOT accepted when requesting leave. See note above for login information.

    SUBMIT REPORT

    2. Enter your ABSENCES IN FRONTLINE

          Please enter the number of days that your plan requires as personal days.  If needed, additional days can be entered as they occur.

    3. Submit a COVID LEAVE REQUEST FORM

    Your personalized COVID Action plan that you receive when completing the self-report process, will include a link to file for COVID leave. Completing the COVID Leave request form will require you to upload a copy of your lab confirmed test results (at home testing is not accepted).  PLEASE follow the detailed instructions on the COVID Leave Request Form page (this page is your LISD username; the form is your LISD Google account).

    You and your supervisor will receive the following emails during the Leave process:

      • Review Notice indicating your application is being reviewed
      • Approval Notice indicating what has been approved

    4. Submit a REQUEST FOR REIMBURSEMENT

    VERY IMPORTANT – When you receive your Approval Notice, you must access the link included in the notice to request reimbursement for your absences. You will complete the Reimbursement Request when you return to work. Reimbursements will be made on a subsequent paycheck. The time of submission and the cutoff date for your paycheck will determine when you are reimbursed. Note: Reimbursement will not occur unless you submit the request.
