COVID Vaccine Clinic

  • For Ages 5-11

    LISD is collaborating with Passport Health to host free COVID vaccine clinics at various campuses for students ages 5 through 11 who choose to receive the Pfizer COVID-19 pediatric vaccine. Parents/guardians must be present for students to receive the vaccine. Insurance information will be collected at the clinic; a state issued ID is required if you do not have insurance.

  • FAQ’s

    Is this for LISD students only?

    In addition to LISD students, children of LISD staff members can sign up as well or siblings of LISD students who may not attend school yet. 

    Can I change the date for their second dose?

    In order to receive your second dose on a different date than scheduled, you will need to register for your second dose with the campus clinic date that works best for you.

    Do they have to register ahead of time or can they just show up?

    Families must register in advance.
