ELEMENTARY HEALTH ROOM POLICY
Bringing
Medications To School
In most cases
elementary school students are not allowed to carry medications with them.
Medications are kept in the health room and administered by the school nurse
or a provider designated by the principal. The child's condition must necessitate
that the medication be given during school hours.
House
Bill 1688 was passed into law during the 2001 Texas Legislative
Session, allowing students to carry and self-administer asthma
medications while at school or school functions with written
documentation from parents and physician. If you are interested
and would like more information on what the documentation must
include,
please contact
the school nurse.
A written
authorization and request of parent is needed for all medications.
It should include - students name, date, instuctions, and a parent
signature. The medication must be in an original container and
labled with the name of the student. Over-the-Counter drugs can
not be given for more than five days without physician authorization.
A physician
authorization is required for all control medications and any
medications that will be given for more that five days.
For
medications to be given by the nurse at school the above minimum
requirements must be met. Please contact the school nurse for
further guidelines or questions.

Immunizations
Required by LISD
Students attending
LISD schools must meet Texas immunization requirements.
If you have
questions or are unsure if your child is fully immunized contact your school
nurse, private physician, or Denton
County Health Department.

Illness
You
should fill out a new health card for your child each year.
IT IS ESSENTIAL
THAT EMERGENCY CONTACT NUMBERS
ARE CURRENT
AND ACCURATE.
Please give
us numbers for cell phones, beepers, etc.
If you have
voice mail at work, also leave a number where a person can be reached.
Attendance
at school is not permited if these circumstances are present:
vomiting, diarrhea, temperature over 100.4, rash, active lice
infestation, drainage from eyes, or the severity of the condition
indicates school attendance would not be in the best interest
of the child. Please check with the nurse if you have any doubts.
Readmission
to school is permitted when the student has recovered or a
written medical
release is submitted to the school.
The
district is not responsible for any costs incurred for medical
or dental care, or for emergency transportation. However, if
you are unable to afford medical care, dental care, eye care,
or medications please talk to your school nurse. There are organizations
within our community that may be able to help. The school nurse
has information on the Texas CHIP insurance program for children.

Heath
Appraisal & Screening
Each year students
in kindergarden, first, third, fifth, and
those who are
new to LISD are screened for vision and hearing.
Students in
second and fourth grade may have a vision
and/or hearing
screen done at the request of the
parent, teacher,
or special education department.
Parents may
request a vision or hearing screen at any time,
simply call
the nurse or stop by the health room.
Scoliosis
and spinal disorder screening is done in fifth and eight grade.
You will receive
a consent form prior to the scheduled screening.
Blood
pressure, growth, neurological, skin, scalp, internal eye,
and other appraisals
are done by referal or on an as needed basis.

you suspect a developmental or learning problem in your preschool
child
you can contact
the LISD special education office.

Nurse
Debbie Kaiser
My
name is Debbie Kaiser, and I am the school nurse at Wellington
Elementary. I have been the school nurse here since
the school opened. I was a school nurse previously
at Old Settlers for four years and Central Elementary. Before
doing school nursing, I was an Emergency room nurse and ICU
nurse for twenty years.
I was born, raised, and educated in northern Indiana. I graduated
from Parkview Methodist School of Nursing in 1973. I also attended
Purdue in conjuction with the nursing school. I am married with a wonderful
husband, two wonderful daughters, and one grandson. My favorite
book is The Lighthouse Keeper. My favorite dream vacation
would be to return to Vancourver Island to kayak and take the drive down
Highway 1. If I could meet anyone in the world, I think it would
have been interesting to have met John Muir. |
