Documents Required for Registration:

• Certified Copy of Birth Certificate
• Original Social Security Card
• Immunization Record (signed by physician) to satisfy State & District Requirements
• Driver's License (or photo ID) of parent enrolling child
• Proof of Residence i.e. utility bill (Contract on home if under construction.)
• Withdrawal form from previous school Copy of Student's most recent report card or school records

These items will be photocopied and returned to you.

Kindergarten Round-Up

Registration: April 14 - 16 (Monday thru Wednesday) 9:00 - 11:00 am & 1:00 - 3:00 pm daily
Night Registration: April 15 (Tuesday Only) 4:00 - 7:00 pm

Attendance Information

Attendance Policy

In order to be counted present, the student must be present prior to 9:30 am. If the student arrives after 9:30 they must have a medical excuse from a doctor or dentist.

Absences

Parents should call the school office each day before 8:00 am to report their child's absence. We have voice mail for this prupose from 4:00 pm to 7:30 am each day. Students need to bring a note to the teacher on the first day back to class explaining the reason for the absence (even if you have called). An excused absence is one which results from personal illness, sickness or death in the family, quarantine, weather, or road circumstance with PRIOR APPROVAL from the principal (ie. absence for the purpose of observing religious Holy Days when it is a tenet of their faith). LISD DOES NOT participate in the National Take Your Child to Work Day! This absence will be unexcused and no make-up work permitted.

Tardies

Students who arrive at school after 8:00 am should report to the offfice to get an admit slip. A student with three or more tardies per six weeks will be referred to the principals office and parents will be contacted. Tardies are considered acts of truancy and unfortuanely it may be necessary for the school to file charges with the legal authorities, if such tardies are persistent. Student who come in after second periond (9:30) are counted absent.
