Minimum State Vaccine Requirements for Texas School Children
Diphtheria/ Tetanus /Pertusis |
4 doses of each or in a series with one dose on or after he 4th birthday. Diphtheria/Tetanus booster every 10 years. (Pertussis optional after 5 years old). |
Polio |
3 doses with one dose on or after the 4th birthday |
Measles/Mumps/ Rubella |
One of each on or after the 1st birthday. 2nd measles required for students born on or after September 2, 1991 and for others by age 12. |
Varicella (Chickenpox) |
Parent history of illness or vaccine on or after 1st birthday if born on or after 09/02/1994 or If born on or after 09/02/1988 and before 09/02/1994 (2 doses vaccine required if given after 12 years old). |
Hepatitis B |
3 doses if born on or after 09/02/1992 or 3 doses if orn on or after 09/02/1987 but before 09/02/1992 Due by age 12 |
