REGISTRATION INFORMATION

New Student Registration Information for 2008-2009

In order to enroll a student at Killough Lewisville High School-North, please provide the following information:

1. An 8th grade report card showing promotion to the 9th grade; OR if during the current school year, a withdrawal form from the student's previous school.

2. Placement/Standardized test results (TAKS test scores)

3. A copy of the student's Birth Certificate

4. A copy of the student's Social Security Card

5. Proper proof of residency
a) A copy of a water, gas, or electric bill
b) A copy of a mortgage or lease agreement

6. Student's Immunization records (see the health office's webpage for more information).
According to the Texas Public Health Department, students moving from out of state or out of counrty must provide complete immunization records in order to begin the enrollment process.

7. Proof of Guardianship (if the student does not reside with a natural parent)

8. Any pertinent personal legal documentation such as divorce decrees, restraining orders, etc.

9. Valid copy of parent/guardian's driver's licence.

Please let the Registrar know if the student has had Special Education Services in the past. A transfer A.R.D. must be complete before enrollment can begin.

Please let the Registrar know if your student has been previously enrolled in the Lewisville Independent School District, so that the appropriate records can be obtained.
