Hi, I'm Jennifer Bullock, RN, the school nurse at Independence Elementary. I am an Aggie with a degree from Texas Woman's University in Houston . My background includes 10 years as a neonatal nurse and 1 year in a pediatrician's office. Illness or Injury at School

Please keep your children home from school for evidence of a communicable disease, vomiting, diarrhea, unexplained rashes or a temperature of 100.4 or more.

I will contact parents or guardians if your child becomes ill or has an injury at school. Please keep your contact phone numbers current with the attendance clerk and school nurse.

Medications in School

Medications are kept in the health room and administered by the school nurse or a provider designated by the principal. All medications must be FDA approved to be given at school.

A parent or guardian must always bring a note with a signature if it is necessary for a student to take medication at school. Please walk in the medicine to the health room in the original container with the student's name on it, the instructions for giving (the dose due and the time it is due) and permission for the nurse to give the medicine. Medications will not be sent home with a student. A parent must pick them up in the health room.

Students are allowed to carry and self-administer asthma medications while at school with written documentation from parents and doctor. If you would like more information, please contact the school nurse.


A physician authorization is required for all medications that will be given for more than 15 days.

Immunizations

Immunizations are required to register for school by Texas state law. You will be notified if your child is in need of any immunizations. For questions or a referral to a clinic for free or low-cost immunizations, please contact the nurse.

Screening

Each year students in PreK, Kindergarten, first, third and fifth grade and those who are new to LISD are screened for vision and hearing. Students in all other grades may have a vision and/or hearing screen done at the request of the parent, teacher, or special education department.

Students in first, third and fifth grade will be screened for Acanthosis Nigricans, a condition that signals high levels of insulin in the body. This screening is done by looking at and feeling the skin on the back of the neck. Contact your school nurse for more information.

Scoliosis and spinal disorder screening is done in fifth grade. You will receive a letter notifying you prior to the scheduled screening.
