My name is Emily Mathew, and I am the School Nurse
at Hebron Valley Elementary School. I would like to provide you
with some information that will be helpful in order to keep the
students and staff safe and healthy at our school.
Illness
You know your child best!! Please
keep them home if you suspect that they are not well!
Students may not attend class with any of the following
conditions:
Vomiting, and or diarrhea
A temperature of 100 degrees or greater
An undiagnosed rash or skin condition
Lice
It is required that your child be fever free for
24 hours without the use of fever reducing medication in order to
return to school. If your child gets sick in school and needs to
go home, he or she needs to picked up in a timely manner when called.
Medication at School
A written request from the parent is required for
over-the counter medication to be given. In addition, the medication
can not be given for more than 5 days without authorization from
a physician.
A signed order from the physician is required for prescription medication
to be given.
Medications may not be transported by the student. They must be
brought in and picked up by an adult.
All medications must be in the original container with the students
name on it, and may not be expired.
All forms for medication administration can be found
on the LISD website, or may be obtained from my office.
Screening
Children in PPCD, K, 1st, 3d, and 5th grade will
also be screened for health problems. This screening may include
vision, hearing, Scoliosis, and Acanthosis Nigricans. Students in
2nd and 4th grade will be screened only at the request of the teacher
and/or parent.
Finally, if your child requires any assistive equipment
they must have a written note from a physician stating that the
equipment is necessary, and that they have been trained to use it.
Equipment includes:
Crutches, walker, cane, wheelchair, sling, brace,
etc.....
