LISD STUDENT HEALTH SERVICES
PARENT INFORMATION
The Health Services
Department has a variety of responsibilities including direct care of the ill
or injured, control of communicable disease, and providing a broad range of
appraisal, educational and health counseling services. Registered nurses provide or supervise
routine and special services.
HEALTH APPRAISAL AND SCREENING:
Vision & Hearing is
routine for new students and grades EC,K,1,3,5,7,&9
and by referral. Screening for Spinal,
Respiratory, Blood Pressure, Dental, Growth, Neurological, Skin, Scalp, Color
Vision, Eye, & Ear, problems may also occur.
MEDICATIONS, TREATMENTS, AND PROCEDURES: Information
and minimum requirements
The parent or guardian must always
provide:
A written request and information including Student
Name, Name of Drug, Date, Instructions, & Signature;
Permission for the RN and legal prescriber
to consult if requested
The prescribing physician or other legal
prescriber:
Must have prescriptive
authority in the State of
Provide an annual written
order and signature for some requests (consult your school nurse for
clarification)
All medications (and treatments or
therapies):
Must be current, in the
original container, and labeled with the student’s name and the name of the
drub
Must be approved by and
consistent with FDA recommendations (refer to PDR) or standards of care
Must be prescribed for or
necessary for administration during school hours
Over-the-counter medications
will require a prescription from a State of
Injections must be essential
for the safety or well-being of the student.
Allergy shots will not be given
Indications & directions
in Physicians’ Desk References will be used as guidelines for
administration of meds
School employees will
Give the med or therapy at
the least disruptive time as close to the prescribed schedule as possible
Not administer a drug or
therapy if it is contraindicated or without appropriate information and
permission
The
following types and categories of drugs and therapies will not be administered;
Vitamin and nutritional
supplements; experimental medications or therapies; doses in excess of
recommended or recognized limits; remedies of unknown content &/or benefit;
homeopathic, naturopathic, or herbal remedies; out-of-date medications;
medications or therapies prescribed for another individual.
ILLNESS OR INJURY:
CURRENT EMERGENCY NUMBERS ARE
ESSENTIAL
School personnel provide
temporary care in the absence of a registered nurse
Transportation is not
available other than that usually provided
Emergency Medical Services
will be called if indicated
The LISD is not responsible
for the cost of medical, dental, or emergency care
Attendance is not permitted
when: The severity of the condition contraindicates attendance; or with
Evidence of communicable disease; or with Vomiting, Diarrhea, Rashes, or Temp
of 100.4 F or more
IMMUNIZATIONS: The
school nurse must approve any exceptions to requirements.
Physician, school, or health
dept. records are acceptable proof of immunizations.
Texas State Law requires
current and age appropriate immunization for:
Diphtheria, Pertussis,
Tetanus, Polio, Measles, Rubella, Mumps, Haemophilus,
Hepatitis B
REQUIREMENTS CHANGE FROM TIME TO TIME (CHICKENPOX WILL
BE ADDED IN 00)
Tuberculosis testing is
required for students entering from any foreign country
Medical and Religions
exemptions have special rules
EDUCATION COUNSELING AND REFERRAL:
Parental permission is sought
for educational programs of a sensitive nature
Confidential information is
shared with school staff on a need-to-know basis
The nurses teach students
essentials of care and prevention during visits
