Nurse's Corner

Hello, my name is Janet Krueger, RN or Nurse Janet. I am the school nurse for the children and staff of Forest Vista Elementary. The role of the school nurse has many different facets. This web site will serve as one of the communications methods that I have to explain my function as school nurse. Along with this web site I would also encourage you to visit the LISD web site for any further information you would like to explain the rules and regulations that will be followed while monitoring your child’s health and well-being.

LISD Health policies

Health and Emergency cards are one of the best ways you as parents can communicate your child’s individual health and medical needs. At any time you have changes in information concerning your child’s health or your emergency contact numbers have changed please print the card and send it to school as soon as possible. We have had incidents where parents could not be contacted when their child became ill because we did not have updated information. All health and medical information is held strictly confidential.

This link will direct you to a copy of the district’s HIPPA policy. Privacy is very important to everyone on staff here at Forest Vista. Please click on this link to the HIPPA policy for the LISD privacy notice to parents.

Policies and procedures help us determine the safest way to provide care to students. Please see‘LISD Health Services Policies and Guidelines for Medications, Treatments and Procedures’ for specifics on what the school nurse and staff are able to do medically in order to make your child’s school experience successful.

Medication administration is one of the roles of a school nurse. Any medications to be administered at school require parent/guardian authorization. Over-the-counter medications can be administered for up to 5 days with a parent note. Please complete and return the OTC or Non-Prescribed Medication or Treatment Request to me with any over-the-counter medication that you would like administered to your child. These medications can only be administered for 5 days then a prescription from your child’s physician will be necessary. Prescription medications require parent and physician authorization. You as the parent and your child’s physician will need to complete the Lewisville ISD Health Services Medication Orders, Parent Authorization and Consent form and return it with the prescribed medications. All medications must be in the original container. For forms please see the following link.

As a special note there are two options for administration of medication for the treatment of asthma during the school day. I, as the school nurse, can keep and administer inhalers/medications or your child can self-administer their own medications for the treatment of asthma with the appropriate documentation while at school. Students are not allowed to carry inhalers at school without the correct documentation. Please contact me if you want your child to administer his/her own asthma medication while at school.

Immunizations or shots are a big part of my role as the school nurse. There are many immunizations required prior to your child being allowed to attend public school. When you enroll your child in a LISD school you will be required to supply the school with a copy of your child’s immunization records. Your child’s shot records will be reviewed by me and you will be notified if they are out of compliance. The Immunization Record Requirements For Enrollment form available through this link will explain to you the immunization policies of this district. Please read the form for further information of the district policies. http://www.tdh.state.tx.us/immunize

In closing I invite you to contact me at 436-713-5194 any time you need to discuss your child’s health concerns. I look forward to taking care of your child while he/she is attending Forest Vista.
