A personal CRIMINAL HISTORY CHECK must be completed and approved before a parent/guardian may work with students or attend field trips. Please complete an application for a criminal history check during the first week of school. The form can be completed online:

1. In your web browser, go to www.lisd.net;
2. Click on “Parents and Students”;
3. Click on “Volunteer Application”;
4. Click on “Click here to begin the application process”;
5. Fill in the required information;
6. Click “Continue”;
7. When finished, be sure to click “Submit”—do not click on the X to close the window.

Please notify the campus office when you have completed the application. The application process normally takes three weeks.
