any new and referred students.
ACN screening for grades 3 and 5. Spinal Screening for
grade 5. Other screenings by referral from parent or teacher.
MEDICATIONS and TREATMENTS
The school does not buy or keep any medications. To give medication or treatments at school the student's illness or condition necessitates adminstration during school hours. A written authorization from parents including student name, date, name and dose of medication, time and reason to give, and parent signature; should accompany any medication sent to school. Students and parents are responsible for bringing all medications to the health room. The medications will be kept locked in there and dispensed to the students as per the instructions. Students in elementary schools are NOT allowed to keep medications with them. Student's may carry rescue inhalers. They should have a current medication authorization given to the nurse.
All medications have to be FDA approved, dose appropriate for
the child, in orginal container with pharmacy label intact. Supplements,
experimental medication or therapies, prescription by physicians not licensed
in USA, doses in excess recommended amounts and mixtures of unknown or unapproved
content will not be administered. The nurse will refuse to administer
any drug if contraindicated.
A physician request and signature is necessary for long term therapies, injections, and controlled substances. Parents have to provide a new physician's order at the beginning of each school year and every time there is a change in dose or schedule of treatment. Doctors may fax it to the school. School needs permission to consult the prescribing physician if needed. Over the counter medication can be given only for 5 days without a physician's order.
