Required Forms

The following forms must be printed, signed, and turned into to the classroom teacher before Friday, August 27, 2010.

1. Please read the Student Code of Conduct and print and sign the Receipt of Student Code of Conduct.
2. Please read, print, and complete the the Parental Consent form.
3. Please read the Student Handbook and print and sign the Receipt of Handbook Form.
4. Please read, print, and complete the Acceptable Use Policy.
5. If your student is new to LISD, please complete the Occupational Survey.
