Required Paperwork
All substitute forms require Adobe Reader. To access this free download, or to upgrade your current version, visit this link: http://get.adobe.com/reader/
A. Instructions
- Open Adobe documents #1 AND #2 (listed below)
- Type your information on the Adobe forms
- Print the Adobe form (if you desire a copy for your files, print two. The forms cannot be saved electronically on many systems)
- Sign the forms where indicated IN BLUE INK.
- Hold your application and forms until you have been invited to the Paperwork Submission Session (Step "B")

B.Hold your forms. The Substitute Department will issue an invitation to a Paperwork Submission Session. You will submit your forms at this session:
The following is REQUIRED for attending the Paperwork Submission Session - NO EXCEPTIONS!
- Original signed forms prepared in section "A", Form #1 and Form #2.
- Bring ID with you as specified in the I-9 Form, last page
- Proof of education - PHOTO COPIES of College transcripts showing 48 hours, Degree and/or Proof of Teacher Certification
- Your best smile! Your photo will be taken for your ID Badge.
