Prior to attending your new hire mtg, please read and become familiar with the
Benefits Summary Information

As a new employee, you must complete the on-line enrollment within 31 days of your employment start date (your first day of work). If you do not go on-line to enroll you will not have insurance benefits.

No enrollment forms are required or accepted; all benefit elections must be completed on-line. To access the on-line Benefits system:

1. Go to www.lisd.net/benefits (bookmark this page for future use).
2. Click on "Click here for On-line Benefits".
3. Click on "Register" to create a username & password. The company key is "lisd".
4. Log into the On-line Benefits system using the information created above.
5. Click on "New Hire Enrollment" to elect benefits and name beneficiaries.

NOTICE TO NEW EMPLOYEES REGARDING YOUR RIGHT TO WORKERS' COMPENSATION

You may elect to retain your common law rights of actions if, no later than five days after you begin employment or within five days after receiving written notice from your employer that the employer has obtained coverage, ou notify your employer in writing that you wish to retain your common law right to recover damages for personal injury. If you elect to retain your common law rights of action, you CANNOT obtain Workers' Compensation income or medical benefits if you are injured.
